Seeing new patients requires that we collect medical history information, privacy statement forms, etc.  All this can be a bit overwhelming when done during your appointment at the office.  For your convenience, Dr. Flannigan has provided download versions of these files which may be printed and filled out prior to you visit.  Just remember to fill out all three forms:

2. Patient Medical History

3. HIPPA Privacy Statement                (Note: The medical History for adult and children are different).
